Information you submit

The coordination form stores your name, chosen contact details, hospital, purpose, preferred date if supplied, short administrative message, consent version and timestamps. Please do not submit medical records or sensitive clinical details. The international patient form also stores country of residence and preferred language with the administrative message.

Who can see it

The practice owner can review and respond to requests through the protected inbox. The private tracking code lets its holder view the request status, preferred date and practice reply; it does not display the submitted name, contact details or intake message.

Messages and notifications

Replies are displayed on the tracking page. No automatic email or SMS notification is currently sent. Direct email links open your own email application.

Retention and deletion

Requests remain in the service until the practice owner deletes them from the active database. Contact the practice with your tracking code to request correction or deletion. This is not an automatic deletion schedule or a guarantee about infrastructure backups.

Other interactive tools

Review marks, checklist selections and the visit question sheet stay only in the current browser page and reset on reload or language change. They are not sent to the practice.

For questions about a request or its information, use the practice contact details below. Do not use this website for emergencies.

The Arabic and French documentary guides search prepared public text locally in the browser, without sending questions to the server or AI or saving them on the site. Detection of sensitive data and urgent keywords is limited.

Educational assistant

Prepared-information mode searches public site content inside your browser. Questions and the visible conversation are not saved by this website and disappear when cleared or the page closes. AI availability is shown in the conversation interface; activation requires the owner to connect a provider. If enabled and separately consented to, only the current question and a previous topic identifier are sent to this site’s server and Cloudflare Workers AI, together with public source excerpts. No full transcript is sent. The model selects sources; displayed medical text comes from the site rather than unverified model prose. Do not enter identifying or confidential medical information. Basic filters cannot detect every sensitive detail. We retain rate-limit counters and daily usage counts without message content; provider processing is governed by its own terms. This feature does not book appointments or replace emergency care or an individual consultation.

Public online questions

After explicit consent, the question and selected language are stored and published publicly for Dr. Eghbali to answer. Do not include names, phone numbers, identity numbers, addresses, photos, documents, or confidential medical information. Published content may be copied, saved, or shared. The site owner can delete a question or reply from the active database; deletion may not remove copies saved elsewhere. Questions submitted privately before this change will remain private. Replies are not individual diagnoses or treatment advice. Contact the address on this page to request correction or deletion. An email address is collected only if you separately opt in to reply notifications. It is stored privately and never shown on the public board. The notification contains only a link to view the reply. A third-party email delivery provider processes the address; notifications are sent after a reply is saved when the service is configured.
